Neptune's Daughter is committed to doing anything we can do in order to provide safety and peace of mind for our guests and staff. As part of this commitment, we require all our guests to fill out and submit the form below along with supporting documents as requested. We look forward to being part of your next vacation!

I confirm that I was fully vaccinated and/or received a booster shot within a period of 14-180 days prior to the journey beginning

Alternatively, I confirm that I recovered from covid-19 within the last 180 days

I am starting the journey with a confirmed negative test result and have not had known exposure since the test was taken

I am informed about health regulations in the destination I will be traveling to and understand all requirements and restrictions involved with this travel

If required or recommended by local authorities, I will wear a face covering over nose and mouth in public while off the boat

I will limit physical contact with people not on my boat for the duration of the voyage.

I commit to report to Neptune's Daughter of any symptoms or receiving a positive covid-19 test result througout a period of 14 days after returning from the trip
